Application Instructions and Criteria

Please gather all attachments before completing the application online. Use the checklist below to ensure a successful application. Incomplete, hard copy and/or hand-written applications will not be considered. NO EXCEPTIONS.

Applicant and Business Information

All sections must be filled out completely and accurately relative to your business as it is today. Only the owner/key decision maker of the company can participate in the MBA.

Code of Conduct and Commitment

Read thoroughly. Submitting your application indicates your understanding and compliance with the Code of Conduct and Commitment.


Attachments must be submitted at the same time as your application. Attachments cannot be submitted before or after your application has been submitted. See below for required attachments.

Financial Commitment

Tuition is $750 per year for the MBA program. Year One tuition is due November 1, 2017. Year Two tuition is due November 1, 2018. Failure to pay tuition by the deadline will result in immediate dismissal from the MBA without refund.

Submitting Your Application

Applications must be received by 4PM on Thursday, August 31, 2017. NO EXCEPTIONS.

Once complete, please upload your application documents in the form below.

Applications received in any other manner will not be accepted or considered. You will receive an email acknowledgment within 2 business days after your application submission. 

Signed application and signed preliminary evaluation with the following attachments:

  • Current Business Plan
  • Annual Financial Statements - previous 3 years
  • Interim Financial Statements
  • Tax Return - previous 3 years
  • 1 Letter of Reference from Business Banker
  • 1 Letter of Reference from Certified Public Accountant (CPA)
  • Current Organizational Chart
  • Headshot Photo (high resolution color JPEG)
